Cruise Plan

The Urraca Cruise Plan, which is prepared by the Chief Scientist, must be on file with the Vessel Superintendent at least one month before the sailing date.

Cruise Participant Questionnaires

At the same time the Cruise Plan is submitted, the Vessel Superintendent must receive from all participants a completed copy of the Cruise Participant Questionnaire for each member of the Scientific Party.

Medical Advisory Service Questionnaire

Although the crew is trained in CPR and First Aid, the Urraca has limited medical supplies and capabilities. Hence the ship has contracted the services of a medical advisory service ( MAS) to provide advice in the event of a medical emergency. To assist MAS in rendering effective service that company has requested that each cruise participant fill in and return to MAS a medical history form. Providing this information is optional but strongly recommended for participants' safety. We also suggest that you bring on your cruise a copy of your Medical History Form so that it is available if needed in an emergency.
